Interpreting medical records- systmonline?

Hi can someone advise me as to this, the 'date entered' for entries on systmonline records, that is the date the information was put onto the system so would be available for the doctors to see?

I don't mean the actual date of consultation which seems to be on the left, Thank you

On any database date entered usually referred to the date on which info was put into the database.

Sometimes decisions may be made and 'signed off' on paper but entered at a later date which I expect would be the date logged. Having said that my experience is in non medical fields.
