Access to work for travel to work - Has anyone used this service?

I'm hoping to go back in May/June after 18 months off. I have bilateral Thoracic Outlet Syndrome. I've had surgery on one side to relieve the vascular symptoms but the neurological symptoms remain.

Commuting is going to be an issue as my face goes numb as does my hand and arm if I drive for more than 20 mins at the moment and my journey is 30-45 mins depending on traffic. This obviously isn't safe.

I've called up today to ask about it and they went through the application with me and are sending it out for me to sign. How was the process for you? I know a colleague gets a taxi to and from work as she has Cerebral Palsy so I know that it is an option if they deem your problems severe enough.

I work for one of the big banks and I will get a full desk assessment from a physio and any equipment I need will be provided.
