Does anyone know if when you change GP, do all your notes pass over or just a summary? Also when a person goes into hospital as an emergency and is a possibility for surgery, would a medical history be obtained from the GP?

Yes when you change GP all your notes transfer - it may take a month or two for it to happen though.
Re emergency, there is not a shared IT system between primary care and secondary care so often in these cases they have to operate without history - they will ask the patient (if they can respond) or family member, friend whoever they can get really. Often in these cases they would not be able to get GP before the surgery had to happen - in really critical emergencies there might not even be time to find out who the GP is let alone contact them. Full details and discharge summary should be sent to GP afterwards though.

Mharhi-not sure what your issues are but if it's concern for lack of privacy... I've changed GPs 3 times in last 2 yrs. Not one of the new GPs has noted 3 important medical conditions requiring medication/monitoring.

The info gets lost in the rest of notes. I doubt they could confirm anything about me unless I insisted they review the notes with a fine tooth comb.

Hi thanks. I am concerned some notes about me have not been passed on to the new GP. I changed last September and became ill soon after. There was correspondance with the Gastro dept at the hospital which would have aided in helping diagnose what was wrong.

Then when I had to go into A and E I wondered as i have just got a copy of my hospital notes, it says Previous history- Nil. Either the hospital didn't check my previous records (at the same hosp) or got the info from the GP.

I have just rang both GPs and got different messages, the old one says the notes go to a patient data place and the new GP says all the notes pass on. I then got hold of the data place and they say the records go on too. A bit confusing.

I'm going to ask for a copy of all my notes from my present GP and see what they have from the old one. I just want to get to the bottom of this and see what's happened.
