I get a really bad pain if I don't go to the loo as soon as I need to (post childbirth)

I just wanted to know if this is just one of the many crappy things you have to put up with post childbirth or not.

Dd is nearly two and ever since I had her if I don't go to the loo as soon as I get the urge to I get really bad pains. It starts at the front (very similar to labour pains) and then goes all the way up my back passage.

I haven't had any incontinence issues, although once or twice I have felt as though I might.

Had a forceps delivery and 2nd degree tear if that's relevant.

Not an expert, but I've had three babies, including a ventouse and 2nd degree tear with the 1st but don't have any pains at all like you're describing.
Get thee to a doc's.
