applying for hospital notes

i am applying for my hospital notes, i have the form from the hospital and the form has countersigniture on it. there is no information to let me know whether i need a countersigniture or not and if i do can it be my dh or does it have to be someone important like when you apply for a passport.
do they make it this complicated so you can't get access to your notes and complain about something?

it's also costing me up to £50- is this normal?

Yes, completely normal. I applied for my ds's hospital notes. I had my hubby countersign and they accepted it just fine. The notes normally take around about 40 days. I paid £10 upfront and then paid another £20 after I had the notes about a month (this was from Birmingahm children's, I'm not sure if all hospitals have the same policy) and they were quite thick (about 2 to 3") and extensive. I was also told that it would cost more and take longer if I wanted his FULL medical records, so for example, his records from our local hospital and dr's office as well as his specialist records.

£50 seems a lot. I recall a charge of around £8 - just to cover admin and the cost of photocopying (and as it was I was never charged).
This was in 2000.

Meant to say, I was told that it could cost me UPTO £50, but it rarely does. After asking around, this appears to be the norm for 2005.

thankyou, i will get my dh to countersign too, i was thinking i had to find someone like teacher/dr etc to sign. i will pop it in the post tommorrow then as it sounds like it might take some time.
