Diabetic high ketones

Hi, my Dh was recently diagnosed with type 2, & he's taking metaformin. Today he's really dizzy, & after he eats he gets cold, sweaty & a bit trembly. Last time (this am) I gave him some lemonade and a biscuit & he had a sleep & he felt a bit better, woke at 3pm blood glucose 17.6, urine glucose off the chart, and high ketones (not the highest, next one down). He ate about 4pm, now he's cold & sweaty & got back in the bed. Yesterday he was fine. The dr upped his metaformin from 1x2 to 2x2 per day yesterday. Should we make the trip to hospital (13 miles) or am I worrying needlessly (its a side effect - will pass, etc)??

High ketones with high blood sugar is a sign of diabetic ketoacidosis which if not treated can be dangerous. You need to go to hospital or at least ring them.

brimfull · 22/01/2011 16:37

wht is his blood sugar now?

tink123 · 22/01/2011 16:38

Tell him not to eat but keep drinking water

Sossiges · 22/01/2011 16:39

Blood glucose is high at 17.6 but was in 20's & 30's before started treatment. Now (10 mins ago) it's down to 14.3

The ketones are the worry. I worked on a diabetic ward and we were told if any ketones with elevated blood glucose we had to call dr immediately.

Sossiges · 22/01/2011 16:40

Hi, thanks for answering, ketones are high but blood glucose is dropping, urine glucose is NAVY blue

I would still at least ring A&E and they can tell you whether to go in, or ring ooh dr

brimfull · 22/01/2011 16:42

blood sugar isn't that high
I would call out of hours doc for advice

Sossiges · 22/01/2011 16:44

we never really tested for ketones of any patients unless they had high blood sugar so not sure what they do about that. Out of hrs doc will definitely give you advice

tink123 · 22/01/2011 16:52

I am diabetic and my ketone test is positive, what should I do?

Contact your healthcare team, and explain the situation to them.

Should the tests show very small amounts of ketones take the following actions:

* Drink water every hour, and continue testing every three hours.
* Do not exercise.
* If your ketone levels do not fall after two tests contact your healthcare team.
* If the test shows moderate or high ketone numbers then telephone your doctor at once, and drink water.

This is off diabetic website

let us know what happens... take care

Sossiges · 22/01/2011 22:00

Went to see OOH doc, by the time we got there he was feeling better, obviously, Dr told him to cut down on the meds, see if that helps. Wasn't too worried about the ketones, said it was due to blood glucose levels normalising...
THANKS for all the advice in hour of need XXX

glad you got sorted. Can never be too careful...

A1980 · 23/01/2011 00:31

Glad he's ok but if he struggling with control why are you giving him lemonade and a biscuit. That's loaded with sugar.....
