Best way to carry a baby when you have a bad back?

Not sure if this is the right forum to post this in, but hopefully someone will have some suggestions.

I carry my baby (currently 18 lb) over my shoulder with him facing me and looking over my shoulder. To compensate for the weight I have noticed that I push my stomach forward and arch my back IYSWIM. I have started to get bad back ache, which I am sure is due to my posture when carrying him.

What is the best way to carry a baby to not get back problems? I need to sort this out before he gets even bigger!
