Also, what would ‘otherwise’ for a man mean, on a death certificate? For example, John Smith, otherwise Jones.

That he was known by two names eg perhaps he had a stepfather's and his real father's names at different times.

If it's UK, it is far, far more likely to be MB (bachelor of medicine).
