HELP! Worms and Trojans all over my face. Wiped the desk top wall paper and messing with everything else...

We have McAfee but scanning shows infections and I have tried various fixes but dire warnings pop up - maily re Win.Netsky and Trojan SPM/LX. Most free scan tools miss them, and I am wanting to know is there a manual way of getting to them??
Anyone??

Have you tried running Malwarebytes? (www.malwarebytes.org)
