Same old iPod nano - new computer - HELP!

I have an iPod nano. Suits me fine. iTunes is loaded on our old laptop. With all my stuff on it. We've just bought a new laptop. I've loaded iTunes on it. But how do I get my stuff that's on my iPod (and on my old laptop) onto my new laptop. PS. I am an idiot and only understand instructions if you assume I know nothing.

Here is a good step by step guide. Depending on the size of your library, you may need either an external hard disk, USB or just plain old DVDR.
