Health Forms

I would be very interested to hear other foster carers experiences with regards to the health forms we submit. I have recently been told that I must complete my section on AH2 and then return to the agency I work for.

This goes against the guidance on the from which states that once completed by the carer, the form is then sent/taken to the GP for their comments and is then forwarded to the Medical Advisor for the IFA/LA…..

Your experiences would be appreciated - thanks

In my experience it has always gone Stratton carer to GP then to medical advisor. Nothing back to agency until the end.

Straight from, not Stratton.

Yes that’s is how it should be but a member of staff from the agency I work for insists it’s not and hasn’t been for a number of years 🥺

I’ve just done mine.
I fill in my bit, take it to Dr, she did my medical ( which was a lot more thorough this time) then she sent it on to agency.

What does the agency policy say in it?

If they’re using CorumBAAF AH form then the instructions are very clear on there.

If they’re using an alternative due to Covid making GP apts difficult (some agencies are just doing self assessments as an interim measure) then they could be referring to a temporary way of doing it. But that doesn’t tally up with the staff saying it’s been that way for years.

GDPR has changed the guidance, so if "it has been done this way for yrs" then suggest they look at this. Should be completed by the GP, sent directly to the medical advisor, who sends a summary only to the fostering agency. Random fostering admin don't need to/ have the medical knowledge to look at the whole medical.

JumpLeadsForTwo · 23/06/2021 20:50

I think some fostering admin want to take ownership of the medicals because they can track when they were sent and chase etc. Also the gp invoices then come to them, so they don't want to pay without knowing they have been completed.

Thanks for your replies all, like many I’m familiar with the Data Protection Act and GDPR. It was the cavalier attitude of “its been done this way since I’ve been here and thats 7 years” that worried me. I am totally committed to fostering, and comply with all however I like things to be done as per policy and procedures… which in this case it seems its not :(..
