Please help me design an evaluation form....

..for a boat event that we're running.

We've got some funding to organise some family morale boosting events whilst the boat is away. The first one will be at a soft play centre. What sort of questions should I put on the evaluation form?

What did you like about the event?

What do you think could have been done better?

Do you think there were the right number of people?

Was it well organised?

What events would you like to see organised in the future?

What did you hope to gain from the event?

Do you think it reached those objectives?

that sort of thing?

i'd also be asking what the best way is of ensuring everyone knows about events, and maybe a space for an e-mail address to be entered so that you build a contacts list?

specifics about catering - adults/ kids etc if you are likely to run another event requiring catering facilities

q's about whether events should be just spouses (ie evening/ day pamper) or family (all kids) oriented - often wives with no kids get excluded from this sort of community event which can cause issues - not sure whether your funding is designed to boost morale of all spouses or whether it is children focused...
