How about a Shoe Box Cake like this. That way you can just mould a shoe for the top (or even buy one of those miniature shoe ornaments I saw everywhere before Xmas).

Alternatively could you opt for a handbag cake instead as that looks easier and there are loads of examples.

Another option is to buy a High Heel Shoe Cake Plate and put a normal/plain birthday cake on it

If you made a rectangular sponge, sliced off the top part for a "lid", covered both bits in rolled out icing and then decorated with either icing pens or the powder/liquid colours that would make the box. It looks like they've used satiny fabric for the lining on this one. You could then mould a shoe from the rolled out icing or, like I said, cheat and buy one .

If you make a shoe you could decorate with anything really as that bit wouldn't be for eating anyway (sequins, crystals, feathers!!)

I read that bit too and thought " if only I had the time to spend a week decorating a bloody icing shoe".
