Right, differences please between toffee and fudge, help

I just made fudge.Except I made toffee

Sugar
Water
butter
condensed milk
vanilla essence
and boiled for about 30mins

Bit of it were fudgy, but the majority was toffee-like
The children on the street liked it, but my teeth doesn't
I wanted fudge

So a few questions,
What is the difference between toffee and fudge?
How do I make fudge rather than toffee?
