How did they do this cake decorating?

I've worked out the general gist of it - square cake, cut a round bit off, stuck at bottom, ice with butter icing. But how did they do the eyes part? All I can think of is an enormous piping nozzle on a massive icing bag, and that seems a bit unlikely!

Do you think using a metal cake cutter as a template and doing a spiral inside that would get a similar effect? NB am very much a noveice at cake decorating!

Look like those Danish shortcake biscuits. Are you sure it's icing?
