Psychiatrist lying in report

I recently had an assessment with a psychiatrist at the CMHT.
I have received a copy of the assessment report. Basically it's full of lies. She's just made things up, things I never said. I worry about the impact of this. Does anyone know how I can get this report dismissed so it isn't on my record? I feel utterly devasted after reading it. I was hopeful to get some help but not anymore.

I'm sorry this had happened, it happens a lot, it's happened to me many times, people stating things claiming I said them when I know I categorically didn't.

It's hard to disprove. But please at least challenge it, in writing. Ask to meet her manager, the original items never gets deleted, but a note can be put on aside it.

I'd contest things. Ask for a recount of what was said prior to x statement. Eg if it claims you eat meat when you are a vegetarian. If it claims you said you did a, but you know you described the story as : I left the house at 8.30 caught the bus and got off at xx.

print the document out. Take your time, don't rush. Highlight say 10 points that bother you the most. Write notes. Then leave it for a day and reconsider. Ask someone or read it.

then go back to the original. Put in the 10 pointers and write a retort in blue.

ask to meet the manager. To express how damaging this mistake is.

Thank you to everyone who has replied. I'm a bit overwhelmed. It's horrible that this is happening often to people. But it is good to "speak" to people who truly understand.
I really wish the notes could be removed from my record.
I am going to look into private psychiatrist help.

You can stipulate to your private psychiatrist that your treatment be kept private from NHS. If medication prescribed this is a different matter.

This has happened to me, not only by psychiatrists, but also, last week by the chronic pain clinic. Factual inaccuracies, eg. Pain clinic said I've suffered pain since April 2024. I've had my condition for over 30yrs. The consultant who dictated the letter has been treating me since 2020!

I've emailed the consultant, GP and PALS pointing out their errors and asking that they update my records accordingly and reissue an amended letter.

Cc to all is a great tactic. Shining a light on stuff can make people perk up and act as they may be embarrassed about people knowing they've messed up.

I agree with cc'ing in other people. It highlights it to many others and embarrasses them.

I did this to school HoY who was very rude to me about my ds2 who kept being moved and sitting next to a girl who had a huge hygiene issue. In the end I cc'd in Deputy Head, safeguarding lead and a couple of others, just to make sure HoY didn't get away with keeping it unknown.

I got an OH report that basically said I was fine and no issues. Nothing I said was in it. I thought I was going mad. I emailed to say you haven't included x and y specific thing (there were actually dozens but these were the key ones) and the Dr wrote back to say I am so sorry I was only looking at the front page and didn't realise there were 4 more pages! So it can happen.

I don’t know, but after DD did a subject access request of her medical records, we did. One trainee HCP had gone back and amended her records, after later events, with the benefit of her hindsight - expressing opinions, not facts contrary to GMC guidelines.
