Mirena coil failed once, can I still rely on it?

I had a positive pregnancy test but tests are now negative. I have had some unusual bleeding.

I finally saw a doctor today and she thinks that the mirena has failed. The strings are still in place but she is sending me for a scan to make sure it's sitting correctly.

She said even if it is sitting in the right place she is still concerned that it failed. She suggested I take the pill alongside having the coil but the whole reason I have it is because I don't get on with taking the pill, so I really don't want to.

If it failed once does that mean it could happen again? I didn't get a chance to ask her at the time. Can I not rely on the coil at all anymore?

Wish I'd thought of these questions at the time but I was really tired (my 20 month old wouldn't sleep last night).
