I've never done across a case in RL, but it does gave a failure rate - usually cited as 0.1%. Which means that for everyone 1000 women relying on it for 1 year, there would be 1 unexpected pregnancy

This is one of the lowest failure rates of all forms of contraception (same as implant, lower than sterilisation)
