performance and doctors letter

have to call doctor back in an hour as have been asked to provide a certificate/letter for tv production company.
doctor wants to know what they need to know and I have no way of contacting them atm.
I know there are lots on here who dance, sing etc, so wondered if anyone can help please?

Probably too late for you now but we just get a general letter from the GP every six months which puts child's details at top and states something along the lines of:

"X is an 11 year old patient at our surgery. He is healthy and up to date with immunisations. He is fit to perform on stage and take part in filming. If you require further information please do not hesitate to contact me. "

I've just had to get one done for a specific job which included being fit to travel abroad for the purposes of filming and work whilst there.

Our doctor charges £10 a letter and the council accepts them for six months from their date.

I've never had to get a letter for performing as dd never did filming & our area don't require one for theatre.

We did have to get one for her current vocational school. The dr was a bit bemused so dh took along something which said to the best if my knowledge dd is fit & healthy and fit to take up full time dance training.

He didn't charge :) partly as he went got school in the sane town as her new school & used to share a school bus!

Spoke to the doctor, she said that dds doctor would do it but it would cost £10, fair enough I suppose.
Can't say what its for yet as we're not supposed to tell yet, but will no doubt have a mini boast thread when we are allowed Smile
