Report to DVLA - how?!

My 81 year old FIL is insisting on driving all the way around a major busy motorway, regularly, in a 25 year old car that we refer to as “Trigger’s Broom” it’s had that many replacement parts. MIL, same age, very unwell, sits next to him and they fight constantly over the ancient, out of date satnav they won’t replace with a maps app.

We have tried lots of gentle and then more direct approaches but it’s just terrifying now. He is physically frail, his eyesight and reactions are poor and he has terribly arthritic joints.

A few years ago there was an online form but I can’t find it - does anyone have recent experience of reporting? Will have to be anonymous because it will destroy all trust and we are trying really hard to help them accept the help that he and MIL desperately need atm. GP surgery is useless and chaotic so I’m not calling them - wouldn’t trust them not to say it was me! As it is, could be a neighbour or hospital worker or someone.

So worried, and exasperated -someone share a link?! Please?! I love them but fk me they are a nightmare!

Speak to the police on the non-emergency number?

I witnessed someone attempt to drive the wrong way around the roundabout and reported them that way. However before they did I was already concerned as they were driving so slowly I wasn't sure whether the car was moving or not!!

How do I report not fit to drive to the DVLA?
You can email the DVLA on Gov.uk, talk to them on webchat or call the DVLA's medical enquiries division on 0300 790 6806 between 8:00am 7:00pm from Monday Friday, and between 8:00am -- 2:00pm on Saturdays.

My brother did an anonymous report recently for my dad. He completed the form online. From the DVLA website. My dad has an appt with his doctor for a DVLA health check. He will report that dad shouldn't be driving as he recently referred him to dementia services after performing poorly on a memory test.

https://contact.dvla.gov.uk/driver/capture-transaction-type I think this is what I used - I’ve been searching for you and it’s really unclear though.

GOV.UK - What do you need to do?

I found the form! Just for others who might be searching. It’s under medical issues, and there’s an option to report someone you’re concerned about. Thanks all for help.
