Fast track CHC - need advice

My father has been given fast track CHC funding - do the hospital organise his care at home or do we? I’ve asked on a FB group I’m on and the answer was that the hospital organises it unless we want a personal health care budget. Can anyone explain in words of one syllable? I’ve got Covid brain fog…

He’s been given CHC funding because he has medical, ie nursing, needs. The hospital will organise it.

Personal health care budget is if you want to arrange and employ nurses and carers directly yourself. Or if you want him to go into a particular nursing home.

I think we want to organise our own care, yes. So who do we talk to about that?

His social worker- hoping he has one? At the hospital social work team if not

There is a CHC team who will do it. Don't do anything yourself. They should liaise with you about it. The NHS will pay for it all (this includes all his care).

If he has got fast track funding, then that is a specific type of chc funding and the chc team will hopefully have some contracts in place that means care can be sorted quickly.

I'm not sure that with fast track Chc funding, that there is much time for personal budgets, although it would be worth asking the question in case they can't find care and you have got the time to ring round lots of local care agencies with specific information about what care you need and check how quickly it can be sorted.
