Care plans - how do we get one?

My DM is currently stuck in hospital because the physio's won't discharge her as they want to put a care plan in place.

Last time she was discharged both her and my DF lied, and 48 hours later she taken back in! So I don't really want a repeat of this.

She told them she had a "private care package in place" which was and is complete bollocks!! What they have in place is a private carer once a week for an hour and my DIL and her friend for a couple of hours to do the cleaning, change beds etc. My DF, 80 with a crumbling spine and spinal stenosis, does the rest as she is 99% bed bound and was only able to get to the loo in the ensuite as it's very close.

I'm trying to put more private care in place, but the lady she uses is on holiday this week. So, how do I get a care plan in place? Or do I just leave it up to the hospital?

If your parents lie and say they've got a care plan in place it will be accepted and she will be sent home. If you put something in place will they cancel it the moment she comes out?

Start by putting in writing the current care “plan” and getting it to the hospital discharge team. Make it clear that you DF cannot cope

Thank you both for your replies.

It's very been taken out of my hands (thankfully) as although the medical side doctors will discharge, physio wont. This is also her fourth stay in 2 or 3 months so they're keen for her not to come back again quickly! So they're putting a care package in place with someone coming in twice a day.

So she won't be discharged until this is in place! That's actually a huge relief to me.
