Teachers and childminders - would anyone be kind enough to pass on an emergency first aid and medical consent wording please?

Just in the process of re-doing mine, mainly for the photo consents and to be clearer on the safeguarding, but need to update the first aid and medical permissions too. Any suggestions welcome....TIA

Is this what you mean:

I agre to my DS/DD receiving medication as instructed and any emergency dental, medical or surgical treatment, including anaesthetic or blood transfusion, as considered necessary by the medical authorities present. I understand the extent and limitations of the insurance cover provided.
I will inform the Group Leader as soon as possible of any changes in medical or other circumstances between now and the comencement of xxx.

I use the word 'essential' instead of emergency just in case a procedure needs to be done that isn't strictly an emergency but is essential e.g. lumbar puncture.
