Can anybody help me with a format/advice on how to write one? I've been staring at the screen for an hour now!

Are you working your notice (can effect whether you need to pay money back / can claim more if you didn't take it in the first place). Otherwise pretty much state that you wish to resign and the date it will take effect. Maybe make some positive comment about your time working there if you want to and thats about it.

Thank you geogteach - I finished work 10 omnths ago (Ds is now 9m) so I wasn't sure of the finish date - basically just said they were lovely (you have to be nice don't you!) but I wasn't going back.

I just said that after my maternity leave, I had made the decision to leave full time work to spend time with my family. Depending on how much you liked your work, you could put nice things about what your time there meant to you and what you will take away with you - I was glad to leave so left it at that! (I never gave a date - was very confused about that one also!)
