College missed an AS result - track showing unsuccesful because offer not met

Ok - DS planning to take a Foundation Degree at a FE College .
Track finally updates at midday today to show him as unsuccesful .
After much phoning he discovers that college have no record of one of his AS results and that as far as they are concerned he's not met his offer requirement .
Hopefully this is now sorted (UCAS confirmed the missing grade to the college ) but does anyone know how this could have happened ?
All DC's exams and results were on his UCAS application so how could the college not have "seen " one of them ?

( and nobody says "whoops ,sorry " or offers to pass me the Ibruprofen I need for my blinding headache )

Im going to portsmouth to do social work, is my home so was very pleased to get in not sure if I would have had to up sticks and move.

Hope all the packing is coming along.

festi that's terrific ,you must be one special person !
Packing - empty house now ,must get on with rest of it .
Nervous about phonecalls tomorrow - hope I can get it sorted .

Can't help feeling it's my fault somehow - it can't really be chance that we always have trouble can it ?
as in ,didn't get into primary or secondary schools without lost appeals and starting after everyone else .
And not wildly unrealistic choices - just your nearest schools ( but inner London ) .
We're older parents ( in our 60's ) and sort of out of the loop ,don't think it helps .

Sleepless weekend and after counting the minutes to when I could phone , found I didn't want to ,felt sick and scared in case was going to get bad response .

Spent 20 mins on phone backwards and forwards to various switchboards with phones going straight to voicemail .

Checked track again and now it has a congratulations message and has been changed to unconditional .

And breathe ....

( DS went camping on Sunday ,hence my involvement .)

Thanks all, ....gosh I'm relieved .
Seems churlish to wonder how it could have happened in first place .....
