I have sold loads before on ebay and I am sure I have ticked the box that says post to UK only.
when I look at my completed listing it says "posts to: united kingdom" so WHY has someone from Italy now won?
expensive item so won't be posting it if I can not track and insure it.
how do I fix this?

Can you find out how much to post it to Italy, then inform the buyer of the change in postage? Then if they aren't happy, cancel it?

just send a cancellation request to get your fees back. Post to Italy is notoriously dodgy.

I second Butterflyspring. I posted to Italy, the buyer completely mucked up paying extra postage charges (claiming they had, then finally admitting they had only paid the charge for UK delivery - but this is just my buyer), then the Italian postal service took weeks to deliver after I posted it as soon as payment was made, and I got neutral feedback about the late delivery and a sarcastic comment alongside it! Angry

Not all buyers from Italy will argue about postage of course, that was just my luck, but the Italian postal service, in my experience, is quite bad and could cost you if parcel goes missing.
