Fertility clinic documents different surnames

my wife and I married in 2021 and she is yet to legally change her surname to match mine and the surnames our future children will have.

can anyone advise whether this may cause problems with the birth certificate if the name on the fertility consent documents are different than our names at the time we register the baby’s birth?

My question is, why would the birth registration have anything at all to do with the fertility documents? You simply register the birth of a child yourself and input your own names. Nobody will demand paperwork to find out how the baby was created. They don't ask for such things of parents who conceived naturally, either.

I can see her potentially requiring proof of name change in future if, for example, your wife tries to use frozen embryos and her passport no longer matches. But these things are still easily solved when you show an official change of name document such as a marriage certificate or deed poll etc.
