Form A - Financial Application

I am filling in the form Notice of [intention to proceed with] a financial application to which the standard procedure applies, and I can find very little guidance online regarding how to fill out this form. Any help which can be provided is much appreciated:

  1. Case No. - Where do you find this? Is it the divorce case number, or do you get a case number on the decree nisi to use? (nisi has not been granted yet)
  2. Nature of Application - my ex-partner and I have no property between us. So far I've ticked lump sum order, periodical payments, pension sharing order and pension compensation sharing order. The others seem irrelevent, but some of the wording is unclear. Is there anything I may have missed.
  3. MIAM - I'd like to claim exemption on the grounds of domestic abuse. This was granted for Childrens Act proceedings. I've ticked evidence of relevant criminal proceedings for a domestic violence offence which have not concluded, but is there any other boxes I should be ticking?

I'm happy to provide further info if people have questions before being able to offer advice.

Hi, I also found this extremely confusing. I put the 16-digit divorce application reference for the case number. I haven't applied for the nisi yet. My solicitor just told me to tick all the initial boxes as to what you're seeking- I did this, but then when I submitted it the financial resolution centre admin called me back and asked if I did really want maintenance pending suit as that would be a further £170-something fee on top of the £275. I didn't really, so I unticked this box and re-sent it.

Not sure about Miam exemption sorry - I was hoping to be able to tick this as I spent
over a grand on mediation last summer while I still stupidly believed my stbx might tell the truth about his finances and actually have a sensible discussion but it all broke down when he turned up disclosing his 'four' bank accounts then failed to ever supply any details for the further five I knew he had.

Anyway, after months of waiting for him to supply details then giving up and going down the court route we were past the six months limit for the mediation form, so I had to fucking pay again for another MIAM meeting which did piss me right off. In the end I felt I'd rather pay and be sure I was ticking the right box than risk ballsing it up and have that delay the process.

you submit it to your nearest financial resolution centre (not your local court, which did confuse me) which might be a ways away. There's a gov website you can search to find it.
