What happens when the person paying receives a bonus. Is this included in the monthly payment for that month?

Depends on how it has been set up.
If a voluntary agreement, then what was agreed?
If using the CMS, then may not matter until next year, as based on past years.

If using CMS and the bonus changes the total earnings for the year by 25% or more you may be able to ask for another assessment by CMS before the annual review is due.

If CM is being paid monthly sounds like you have a private arrangement? Usually CMS calculates a weekly amount to be paid and issues a payment schedule for a year until the next annual assessment.
