Did some googling for you.

The court would have destroyed your records by now. The link is some word document.

The courts should have kept your records, try and request a copy of your consent order. It will be easier if you know your case number which will be on your absolute. The D81 is just the info form that goes with a consent order.

I was divorced in 2001 my daughter was 17 and in 6th form education.
On the consent order I was entitled to maintenance of 3558.06 per year while in full time secondary education he would not pay me anything and said he would get the police on me if I asked again. I have MS and was truly worn out s with the divorce and so we just struggled on. In the September my daughter went to university in our home town so she lived with me to save any costs.in the consent order it said he would assist with her secondary education to be agreed with my daughter and him directly she did asked him for help but he refused I did write him a letter asking for some help but he refused and told me to sort it out myself.
In 2009 my daughter had her first child and I asked if she was going to be ok financially she said she would be ok if she could pay off her student loan.
I went to the bank and took out a loan for 5000 and paid the loan for her.
I never asked my ex husband for a penny.
In December 2018 I was looking through some old papers and found a Scottish widdows policy in joint names I phoned them up but they advised me the policy was cashed in September 2015 I knew I had not signed the form and certainly not had any money. The long and short he had committed fraud my daughter contacted him and he admitted what he had done he begged her to ask not to get the police involved and he sent me a cheque.
On the bank statement pre divorce there were 7 direct payments made to Lincoln now sun life of canada and I asked if I could see them in case I was due anything else he said yes but now I have been sent a solicitor letter saying they were critical illness payments ? There is nothing about his pension and nothing about a vast amount of money he was holding in his name for his father
