I'm totally consumed by panic and fear, I have a long term Chronic illness, haven't worked for 7 years because of it and now my marriage of 16 years is over. I just don't know what I'm going to do, where do I start? I have a 16yr old and an 11yr old to protect from all this, but I'm so stressed about having to leave my home, and no idea what my future is going to be, I just feel sick.

Same boat here very soon, first is to get some legal advice as it may be possible to stay in the house until the youngest is 18 also if you have brought up the children and not worked you maybe entitled to more of the house equity than your ex. I have my fingers crossed it is the same for me.

You will but need to do some homework first. Can you speak to the council ref housing. I know how little stock there is. But you might be considered a priority.
Look for Information regarding benefits.
And if appropriate a conversation with social services about your care needs and if there is any help for you.

Also worth looking and the disabled parent network for advice and information.
