Decision Support Tool (DST)

Social services have decided to undertake a DST for my DM to see if she is eligible for NHS continuing health care. She has been in her care home for seven years and is in the final stages of dementia. We want her to stay there for end of life as it’s a lovely home and she’s very settled there.

This DST hasn’t been requested by us, but is going ahead. We’re nervous that, if it is decided that the NHS should take over care decisions, they may want to move her.

has anyone been through this?

Who is currently paying for the carehome, the staff will look at her care needs to see if they are social or nursing needs which will help determine if she is entitled to funding. It doesn't mean the NHS take over her care or that she will be moved. They may also be looking at fast track funding. Does anyone have power of attorney for her. A family member is usually invited to part of the meeting so do ask if you'd like to attend.
