I did one years and years ago, don't even think the internet was invented then. I need to get medical records for my DD, it's mainly dates and procedures since she was born.

It says i can email but not sure where, who and will they give me my child's information

You will need to get your id and your childs birthcert stamped and witnessed at a garda station along with the FOI letter. List of foi forms here :

@Wendysfriend do check what is applicable to the country/jurisdiction you are in as it maybe different from England.

This isn't a Freedom of Information request, this is a data subject access request.

FOI could be the correct term!

I had to get some medical results from a Dublin hospital through their subject access request process. If you google the hospital name and subject access request you might find the how to.
