Face coverings exemption

I need to get an exemption letter to fly without a face covering due to a medical reason. Contacted GP who said 'please see the government website regarding the exemption for not wearing masks,
face coverings and exemption cards and download the exemption card template'

Am I being stupid as I can find no template? I can find a reference to it on the gov.uk website but no actual template

Most airlines insist on a GP exemption letter rather than a self-cert which is basically what your GP is telling you. I doubt many airlines would accept one. I've flown with 4 airlines and, checking which type of masks they ask you to wear, have always spotted that exemptions need to be certified by your doctor.

Thanks @PAFMO but GP I took it was alluding I need to complete this form before they will do the letter. I've emailed doctors twice now but just get the same response
