Hi

Does anyone’s Covid Pass nhs (app or letter) have their middle name on?

Travelling tomorrow and just realised my passport and Spanish health form need to match so both have middle name on but nhs vaccine proof doesn’t

Anyone had issues?

App is in my married name & all my passports are maiden name. This won't be fun...

Normally you're alright if you can tie documents together, birthdates match for instance.

@lljkk - if you contact your GP surgery and ask them to change your name back to your maiden name, then your NHS app will match so your covid pass will match passport.

My GP surgery just did this for me but I had to show them my passport and explain why (I got married fairly recently and just hadn’t ordered a new passport yet)

The app updated within two days when they changed my name back.

Can't answer the question OP but have the same issue. I have also tried to 'receive an offline copy by e mail' so that I can print it off (contingency in case of phone problems) but the button on the app doesn't seem to work...

Yes that button didn’t work for me

I got a paper version from nhs online which took about 5 days

It was fabulous! No one cared it didn’t have middle name.

Basically all your vaccination, pre flight tests (if not vaccinated/kids) Spanish health forms etc are checked at the departure airport so mine were aall done in Manchester (inc Spanish health form)

On arrival in Spain all they did was check passport and leave you to scan the QR code on the Spanish form on a machine - they didn’t check it or look at any tests or vaccine certs

When leaving Spain it was Spain who checked the uk passenger locator form, the vaccination certs (to avoid isolation on return) and the negative antigen tests to return

Uk only checked passport on arrival

So arrival destinations do very little it’s all done at departure end

Information those in maiden/married name it’s the uk side that approve by look of it not Spain etc so if Tui or whoever say it’s ok then it will be I imagine

Ibiza was heaven!

My passport is in my maiden name and my NHS certificate in my married name. No one asked at either UK airport or in Cyprus airport. I had a copy of my wedding certificate just in case.

Just wondered if you completed your Spanish locator form to include your middle name and said your Covid pass had your middle name also (when it didnt).
Our Covid passes dont have our middle names so I completed the locator form to exclude middle names completely but wondered if this was right?
Stressed as travel first thing Tuesday and its now Sunday night.

I saw on another forum that the NHS Covid passes don’t feed through the middle name detail from your NHS records, which is why it’s not on the certificate.

However, if you go into the “your health” bit and “GP Health record” on the app, it should show your first middle and last name (alongside other details. So if you were to have difficulties (unlikely as it seems to be system issue rather than individuals, so we would have heard about it by now if people were being turned away) you could show that alongside the QR code etc. On the app.

I made my Spanish form match my passport I think so inc my middle name

It won’t matter either way - it’s the uk end which check all documents - not Spain and uk don’t care much for middle names

Once in Spain all you do is scan the we code on a walk thru monitor - no one physically looks at it

@lljkk I just went to Spain and have the app in my married name and passport in my married name. I took a photo of my marriage certificate (because I couldn't find the original!) but they didn't look twice at my app name. 🤷‍♀️
