Spanish health form

Flying to Ibiza tomorrow and just realised

My passport has middle name
Therefore my QR code for the Spanish health form also has middle name because they must match
My nhs vaccine cert doesn’t have middle name

😱. Anyone else have this?

Plenty of threads about travel on there, also people who live/travel outside of the UK.

I have a similar problem as I live in France and they record everything in your maiden name! Luckily my residence permit has both names, as does my vaccination certificate.

We're just giving this thread a bump for the OP in case anyone's around who can answer her query.
