Does anyone know if there's an on-line form to fill in to request this, or will I have to hold on the 'phone for hours?

You can request one via the NHS app.

You can get a paper one, or if you do have the records on the app and just want a copy that doesn’t rely on battery life and connectivity then you can generate the PDF and print
