Pre vaccine screening text

I’m due my second vaccine next week and have just had a text with a link to a pre vaccine screening form.

I have to put all my info in, just wanted to double check this isn’t some sort of scam?

Sounds dodgy to me, I definitely would not be filling in any details on that. Not unless it was in the same text message conversation chain as other messages from my GP surgery maybe. I dont think anyone I know has has a similar request.

Sounds dodgy to me too. Nobody I know has had a form like that to fill in. All of your info is on your medical record, they don’t need to ask for it.

My Mum booked hers via the national online system. She just had supply information when she got there both times.

Mine was booked at my GP surgery,. They had pre-printed forms waiting for me both times - all filled out ready, I just had to check them.

Sounds dodgy to me. Can you check with the place where your jab will happen?

I haven't heard of that, and it would be unusual to request sensitive health information in that way. You need to check with your GP if they are doing the vaccine, or if not contact the vaccine centre or 119.
