Sick / isolation note beyond 10 days

If an employee gets Covid they can print an isolation note from the NHS website to certify their absence for 10 days.
If they are still unwell after the 10 days isolation, can they download another isolation note or do they need a doctor’s note?

My employer said could self certify for 7 days after that (temp not down for 48 hours)- then would need doctors note
