What semen info do I need to take to hospital?

DH is going to gp this week to pick up semen analysis results. I have to then take results with me to hospital.What info does dh need to ensure he gets for me to take with him?

The GP should give him a print out of results after deciphering it for him - at least this is what my GP did.

You also need to take a list of any medications he is on - if any - because he won't remember at the time and it can make a difference.
