I was diagnosed with severe ptsd and trauma with my ex partner as I was consistently beaten and graped with no consent and coercion. When I have finally managed to break feee I notice that I am pregnant and was too late to abort so I just kept them. I have never lived with him he’s always been offf with another woman but finds time to bully me. I now am due in December and I am wondering whom I can give my form to as I don’t like to disclose detailed information as to my shitty past. I feel as if the midwives are unaware of this form the only other people I can think offf will be my gp or maybe my previous talking therapist as he is aware of everything. What can I do? Also when I do post the form to them am I expecting payment straight away or will I have to wait a few months. I put it on my journal and they said that I will be receiving more money due to this form and post it when I’m ready.

Just ask them in journal for the address and post it to them when you're ready. The sooner you do it presumably the sooner they can increase your payments. Make copies of it to keep in your own records.
