Hey ...

Sooo .. im clueless
Please bear with ..

2 scans last week ( transvaginal included) showed a - small fibroid -

Appt with my nhs gp tomorrow pm..

Obv im gunna ask more re its location / positioning..

Given that my partner & i are hoping 2 concieve - - is there anything in particular i should ask ?

Additionally - i had some hormone & fertility tests done privately - - should i be opting to speak to a private gynacologist re this etc or ?

I worry as im 36.5

( read so much about fertility decline after 30 / 35 )

And also likely pcos.. although as corpus luteum was seen on scan - - does this def mean ovulated that cycle at least ?

Many thanks 4 any tips / knowledge

I have PCOS and a fibroid, done lots of research! You need to get in inositol; I was on inofolic alpha, which includes folate. Also, vitamin D, and COQ10 should improve egg quality.

Re. Fibroid, you'll want to know the size, and whether it's blocking anything. They are mostly harmless but can affect ability to conceive if large or blocking.

Ask about getting an HSG which tests for blockages.

Ask about getting day 21 bloods to test if you're ovulating, and also AMH to understand more about egg reserves.

Also ask for semen analysis for your partner - so often ignored in fertility but is half of the process!

Hope this helps and doesn't overwhelm

@worldwidetravel2017 re. The private fertility tests; make sure they have your nhs results and vice versa. Continue probing with NHS as there are some scenarios that get prioritised. Ask about wait time for referrals to fertility clinic, find out which clinic the referral will go to / and call them separately to understand what they say wait times are like for nhs referrals.

I'd keep the private clinic in the picture; nhs wait times can cause so much stress (I waited an additional 18 months on top of previous wait, due to covid) and it drove me insane. If you can afford private, depending on wait times, it might be worth going down that route. Check on your entitlements based on your health authority; some will allow you to get nhs treatment after trying privately

Also with pcos, you can have corpus outrun form, but not necessarily have released an egg, or egg quality might not be great

A fertility MOT for conception normally includes an HSG or hycosy. Both look at the shape of the uterus, checks the size and location of fibroids and also checks if the tubes are clear. My understanding is that HSG is done using xray, whereas hycosy is ultrasound. I had the hycosy, and personally would prefer not having the extra radiation around your uterus/ovaries!

I have 4, small fibroids (1-2cm each). They said though that the position of them would not affect fertility.

They are more common if overweight and in certain ethnic groups. Ensure you are a healthy BMI, which will also help fertility and hormones.

I would get the book 'it starts with an egg'. I got one from ebay for about £4. Its a useful read and some simple lifestyle changes might help your chances of conceiving. Start taking folic acid months before actually TTC. best of luck.
