Letrozole and Low Sperm Count

I'm starting my first cycle of Letrozole this week alongside Metformin.
My Doctor said we are going against protocol slightly as my husband's sperm count is 13 million instead of the expected 15 million. However his motility is excellent and his morphology is normal. The count has doubled since last year.

Has anyone tried Letrozole with a lower than normal sperm count? I have 3 months of this, then a review which could determine further Letrozole or IVF.

@MrsA2020 do you have any issues on your side? We have unexplained infertility and I've just had my first month on letrozole. I'd think that it's worth a shot, and maybe why your doctor is doing this, as it's a cheaper option than IVF and a good last resort. I feel like my first month has been unsuccessful as I have all my usual PMS symptoms, so it's a bit gutting after you allow yourself to get a bit more hopeful. I hope things go better for you!

@BritInNZ Thanks for responding! All my blood tests and scans came back fine, the only issue he saw was the fact my cycles are irregular. I'm sorry to hear you don't think it has worked this time round ☹️ I hope this isn't the case for you though! How many cycles did your doctor give you?

@MrsA2020 my first round on letrozole was successful and I found out I'm pregnant a few days ago. I hope you got some good news too - and if not, I hope it's just around the corner!

@MrsA2020 just wanted to add to this... I presume you mean sperm concentration rather than total count of 13 mill (based on the guidelines being 15million / ml). If so, I got my BFP last week after over a year trying and being 2 months from starting IVF and my OH has a lower concentration than your OH. Keep going, be positive, it might happen!!!! 💕

If so, I got my BFP last week after over a year trying and being 2 months from starting IVF and my OH has a lower concentration than your OH. Keep going, be positive, it might happen!!!! 💕

We've got very low count - can I ask what your partners results were for motility/TMSC etc if you know them please? I think I'm looking for some hope 😟 xx

We had way above average motility and morphology at the last SA in March and the overall count was 40 million I think. I think the motility was around 69% and then broken down into the different types but all that was positive. Morphology was 9%. Because we were resigned to starting IVF, he hasn't done another SA since the end of March so I don't know if any of the parameters improved. We were told that it was absolutely possible to still get pregnant with those numbers but it would take an unknown amount of time basically - sort of how long is a piece of string situation. Where there is sperm, there is hope so keep DTD and you might get there. I had stopped tracking completely too, literally we had given up. X

Thank you! Did you do anything to increase the numbers?

I'm not sure if ours can be improved as they are all so low 😟 we are going to see Dr Ramsay in a few weeks x

The board line number didn't change with supplements, but the morphology just shot up.

By the time we got a positive, he has been taking Proxeed 2 daily, vital DHA, ubiquinol and pycnogenol for 5 months. As I said, no SA in the last few months so not sure if numbers now but between Jan and March that element didn't change. I'm sure Dr R will give you lots of advice. @HayMay

@HayMay not sure if it helps but my husband's count (or whatever it is) nearly doubled. His morphology went from 3% to 4%. His motility was always 76%. He was just taking wellman supplements for a few months before his second SA and then he also cut down on alcohol, improved diet and less hot baths (as he loves them!) we think this helped his count or concentration nearly double.
