Rising HCG but nothing on scans Help!

Hi Guys, I hope someone can relate or help.
I am really early I believe about 4 weeks, however I’ve had 3 bloods taken from me, HGC started at 115-131-147 every 48 hours. Midwives are saying they can’t see anything in my uterus but they can’t rule out an Ectopic Pregnancy, I’ve been bleeding for 6 days now and it’s started to calm down. The first two days was heavy bleeding with clots and a lot of pain. I still feel so weird and I still have a background pain in my left pelvic area. They’ve told me to come back in a week! Which I find worrying if they suspect a ectopic, I have the worst paranoia and worried I’ll have internal bleeding if they don’t find it quick enough, am I worrying myself too much? Has anybody had a miscarriage with slow rising HCG
Please help I’m so worried and should I go back to work? as I’ve been off for a week with a suspected miscarriage
I feel ok but I still don’t feel right, my first pregnancy so I know my body is meant to feel a change but I just don’t feel the same. The worst thing is my wedding is at the end of the month, everything paid & booked. Just so worried and scared
Did everybody with ectopic feel pain. I get minor sharp pains throughout the day in different areas

I think with ectopic pregnancies the pain is on your side mostly. X

Sorry you're going through this, it's horrible. With my ectopic I had similar symptoms to my other miscarriages at first with cramps and bleeding - the pain wasn't only on one side at first but further down the line it was very much on one side. It sounds like they are monitoring things which is good, maybe ask if you can have a scan any sooner as you're feeling anxious? X

@Miabellaa I'm so sorry! I had an ectopic in June and I had bleeding as well, but no pain at all - at least nothing major that felt off.
I know it's scary, but if it's not visible (yet), it isn't big enough to affect your tubes. But at the same time I wouldn't want to wait a whole week until the next appointment, I totally understand you!

@Bundaberg84 thank you! Hearing that relaxes me a lot! So thank you💕 I just hope it don’t rupture if I am having an ectopic. If I feel bad pain I’ll definitely take myself back, if you don’t mind how many weeks were you when you found out your having an ectopic pregnancy and what did they do

If you’re only 4 weeks, then the baby’s too small to see on the scan yet, anyway. You’re not going to see a baby there before 6 weeks even if it’s in the right place. If the BHCG is rising then a miscarriage is unlikely. They’ll want to play it safe though, hence the reason they’ll keep scanning.

I was 5 weeks and a couple of days when they knew something was off. My hcg levels doubled though, they were on a normal level as according to how far along I was. I had to remove my right tube at 6 weeks exactly, it was already 4cm big. No rupture though thankfully and no pain.
I could have opted for methodextrate injections but the chances of still having to have surgery were high, so I just wanted it to be over fast. I healed very quickly and are already back at TTC.

Hi @Miabellaa - I just thought I’d add my own experience as you asked if anyone has had a miscarriage with slow rising HCG. I did, 3 years ago. I miscarried at 7 weeks, but it’s likely the baby died earlier than that and my body just took a long time to realise/accept it. After I miscarried, I continued to bleed for 2 weeks and my HCG continued to rise, but very slowly, just a bit at a time. It took a month for it to completely go back down and I had to go in for blood tests every three days because the hospital were concerned it was an ectopic pregnancy. They also said if I experienced sudden pain to get there ASAP. I was fairly certain it wasn’t ectopic because of the amount of blood etc (tmi) I lost in the first couple of days. Unfortunately it seemed my hormones were just quite a mess!

Hopefully, hopefully you are just going through a horrible start to your pregnancy, but I wanted to reassure you that if you sadly are experiencing a miscarriage, it is possible for your HCG numbers to take a few days to catch up with what’s happening, it doesn’t necessarily mean it’s ectopic.

@Greenstick - I understand, thank you but they told me sometimes with the vaginal scan they are able to see the sac early as 4 weeks and as they’ve done it twice now with second opinions from sisters of the ward, they can confirm it’s an PUL

@FeeneeTTC Hey! thank you so much honestly, the midwife said that to me today! that my body may not have realised and it may come down, as I said they scanned me and couldn’t see anything! & unfortunately I have accepted this is a miscarriage as they told me my slow rising hcg isn’t a good sign, also when I was in hospital the amount of blood they saw confirmed that it wasn’t a good sign they made it clear it’s either and EP or a MSC. However Thank you sis for sharing your experience. I hope all is well now💕

@Miabellaa it was scary, but also made me feel grateful to live in this day and age where things like that just mean a short visit to the hospital and being fine again. Whatever it is you're experiencing, you'll get through this!

Thanks, it's been really hard but I was lucky to have 1 successful pregnancy in the end. With my ectopic they couldn't find it on the scan at 7 weeks, I was 8 weeks when they could see it in the tube. Yes HCG levels rose every time but usually not as quickly as they expected. The only exception was my last mc where they were through the roof, they ended up finding two sacs that time which explained the high levels. Hope you're doing OK Flowers

Sorry to hear you’re going through this.
I had an ectopic last year, at 5weeks. I knew nothing of it until waking up at 4am at 5weeks with right sided pain, it was in my groin. I started feeling sick, getting sweaty, loose stools, sharp pain in my bum (could hardly sit down), was crawling around the floor, HR was 140, paracetamol not touching it. I wasn’t sure what to do, so called work to speak to a doctor (I’m a midwife so was very lucky), and they suggested coming in to A&E, which I did. Had a scan later in the morning to find I was full of blood and a partially ruptured ectopic. I had surgery a couple of hours later and lost my R tube.

The pain of bleeding internally is quite something. I don’t think you wouldn’t realise.
I hope they get to the bottom of it soon. Rescanning in a week is fairly normal, but you go back if something big changes

My ectopic I found out when I was bleeding, scan showed no baby in uterus and my bloods sat at 178 at 5 weeks. Took 4 people to find the ectopic in my tube. Between diagnosis and mtx shot was 2 weeks and during that time I had the most severe horrific pain. I was crawling across the floor trying to get to a phone. It still didn't rupture at that point.

If you get pain in your shoulder, side, unusual bowel movements or pain when you pee or poop get straight to the hospital OP

Sorry you're going through this OP.

With your HCG levels in the low hundreds, you won't see anything on a scan. I think they need levels to be c1500 or something close to that for anything to be visible.

Obviously you know your blood levels aren't increasing properly - that was the way my ectopic was diagnosed. My levels never exceeded 500. They would go up a bit and go down but basically stay pretty level. I was treated with methotrexate.

I don't recall feeling any pain at all with my ectopic. I had bleeding constantly though. I think the pain you really need to be worried about is shoulder tip pain and I think that happens only when a tube is rupturing (obviously you do not want to get to that stage). My understanding is that your levels would have to be significantly higher than they currently are for that to happen.

I hope you get some answers and treatment soon.

@moomin11 thank you, I hope I get better soon was seen by a gynaecologist last night as I still had pain in my pelvic but she practically turned me away, took my bloods and got sent home with codeine, still feeling quick sharp pains, told them my bowels aren’t the same and in pain when I go but they ignored that. This morning EPU called and said I need to come in to see a consultant tomorrow so I hope they will sort it. X thank you

I’m back tomorrow so definitely will get an answer as I’ve been in pain for a week now and been bleeding for a week. Sad that I have to keep going back and everytime no answer still feeling sharp random pains even if I take a deep breath.

@kirinm Hi, sorry you went through that. was they able to diagnose that without seeing a live pregnancy in the tubes. How comes you went for the injection is that because you was early, also how long was your recovery xx

They did eventually see a mass which they concluded was likely the pregnancy but my levels didn't drop on their own. They didn't move for several weeks and so the hospital wanted to end the pregnancy. (As did I after that long). You can only have methotrexate if you have low levels - I think it's below 2000. I don't recall feeling unwell but mine was a long time ago. Even with the methotrexate it took a while for my levels to come down. I was just about to have a second shot when they finally started moving. It took about 3 months from finding out I was pregnant to finally getting my hcg levels below 5.

I'm glad you're being seen again. I was turned away and assured it was not an ectopic without even being examined and a couple of days later they scanned me and diagnosed it. Let us know how you get on xx
