Too low progesterone levels? Can it differ from month to month?

Hi all. I'm new here. Just wanted to ask some advice. I had a CD21 test in my last cycle to check whether I was OVing. It came back all fine and with a reading of 64. I have now started to see a private gynae who checked so that my bad period cramps were not related to endometriosis (which he has concluded it is highly unlikely, which is great). As I am wanting to conceive he also ran a CD22 test on me. This came back at 21.5!!! I am floored. How can it differ so much from one month to the other. Also, he said that at this level it is not likely that my body will be able to sustain a a pregnancy! and has put me on an Italian fertility drug called Neoclym, which is to boost ovulation. I am so, so confused and really don't know what is right and wrong. Scandi

Have you been given a diagnosis of the underlying problem?.

Progesterone levels are only part of the story; have your LH and FSH levels been both tested (this is often done early in the cycle) and compared against one another?. This should be done as a matter of course.

Would like to know how this gynae concluded that your bad period cramps are not related to endo. I ask this as endo is often only diagnosed through a keyhole surgery op called a laparoscopy - its not usually detected by other methods.

Pinkali37 · 17/01/2008 11:25

Hello

Sorry to butt in but I have severe endo and was diagnosed by laparoscopy, have had 2 ops since then and am desperate to conceive. Consultant suggested Clomid so following last op waited for period and to no avail. Took test and was pregnant but miscarried at 5 weeks (P level too low) (shocked that it worked without help). Anyway have have just finished 2nd course of clomid and due for blood test on Friday to check progesterone.

I did not have classic symtoms of endo and you must fight for a lap to get diagnosed cos I had numerous scans and nothing showed up.

Hi there

I hope nobody minds me jumping in.

I just got a real shock when this morning I found out that my progesterone level on my day 21 bloods was a mere 9! That is stupid low! I mean 9 !!!!!! How on earth will I be able to conceive!

Have had day 3 bloods and also an ultrascan. They suspected PCOS as well but it hasn't been confirmed or ruled out as my right ovary shows signs of possible PCOS.

Ohhhhhh why is it so hard!

I still haven't been perscribed anything!

I would think PCOS will be confirmed I am sorry to say.

Am a PCOSer myself so know a lot about this particular condition. Anything you particularly want to know?.

I would think the day 3 blood test will show an imbalance as well between LH and FSH - ensure that these levels are compared against each other rather than looked at separately. In many PCOSers there is an imbalance of LH to FSH. A polycystic ovary can look larger than a "normal" sized ovary (which is about the size of a walnut).

Have you been referred to a subfertility unit as yet?.

www.verity-pcos.org.uk is the website of Verity and would suggest you look at this.

HTH

DungunGirl · 18/01/2008 11:14

Thanks Attila!

To be honest I just want answers now. Good or bad!

I def have other signs of PCOS....I have been gaining weight for no reason! No increase of food intake or reduction in activity! If anything I am more active than before!

I am having my day21 bloods repeated and they are going to look at progesterone again and also FSH and LH levels then.

Have been referred to subfertility clinic today!

Pinkali37 · 18/01/2008 11:34

Can they just not put you on Clomid and monitor you closely?

DungunGirl · 18/01/2008 11:35

I hoped my GP would do that but it seems the Gps in my surgery/area are just too darn straight!

They all like to refer to the hospital and let them do it!

Pinkali37 · 18/01/2008 11:43

SHOUT at them !!! Who are they to control your life and future? It makes me so mad. I asked a locum doc if they would help and he straight away said "no", then once my consultant suggested I tried it my doc was fine at prescribing it and even suggested it the day before I went to see my consultant. A friend of mine who has the same problem as you was prescribed it by her doc as though it was buttercup syrup.

DungunGirl · 18/01/2008 11:45

To tell you the truth I was thinking that it would be easier to order clomid off the net...just the 25mg one. Until I get seen....

I threatened the Gp that i would do this but he just said it was best to wait.

ARRGHHH!!!!

DungunGirl · 18/01/2008 11:46

I think it is just so unfair how some Gps will perscribe it without a second thought and others would rather be shot than perscribe it without a consultants orders!

ARRGHHH!

primrose88 · 18/01/2008 13:04

DUNGUNGIRL

I have left you a message on taking clomid for the first time scared and excited thread!

With a level so low you really should be percribed something. I wouldnt advise you to get any drug off the net(you dont no where it is coming from)

Go back to your doctors and bang down doors, ask him why he wont perscribe it to you he must have a reason, I no its a powerful drug, but by what your saying your a perfect candidate for it.

Do you think maybe(just maybe)... your pro levels are 2 low and he feels clomid wont help you which is why he has reffered you to the hospital.?????

I wish you so much luck I really do. I understand how you feel.

primrose88 · 18/01/2008 13:08

dungungirl

Or maybe he feels you require further investigations

AttilaTheMeerkat · 18/01/2008 13:10

DG

If they give you clomid you must be monitored closely whilst on it and certainly not left (that's the problem with GPs prescribing it - they do not have the time to monitor you). I say this as it is not always suitable for all PCOSers to take (clomid resistant can and does happen).

I actually was not given clomid due to my sky high LH levels (clomid can encorage further production of LH and this is why it sometimes does not work for all PCOS patients).

You should be on this for a maximum of six months after which time other treatments should be looked at (these include injectible drugs and diathermy surgery).

Please don't order clomid off the net no matter how tempted you are - many well known drugs are counterfeited. Also if you take it without any monitoring you won't know whether its doing the job its supposed to or not.

Pinkali37 · 18/01/2008 13:32

I agree !!

Whilst on clomid you must be monitored, hyperstimulation is a common side effect (especially for PCOS sufferers) and it can be fatal.

Don't buy it off the net, just put your foot down and even threaten to report them to their higher body (i.e. Medical Council thingy).

Clomid is for a max of 6 months because it can apparently cause (don't think that's the right word) ovarian cancer !!

Pinkali37 · 23/01/2008 11:32

Just telephoned Doctors for blood result and apparently it is at 1.0 NOT GOOD. Does anyone think this could be becaus eit was actually day 20 and my cycles even on clomid are around 33days? HELP feeling really low

jd32 · 15/02/2008 15:23

I have been trying for a baby for 2 years, got diagosed in June 07 with Endometriosis, had a laparoscopy & feel pregnant in August, had an m/c in October & been trying ever since. Had results of day 21 blood test yesterday, level was 31.5, I know that it has to be over 30 to be normal but have been reading that this is still quite low & should be above 50! I would like to try Clomid but not sure if this is necessary, would it hurt to try it? I am so confused!

WorzselMummage · 15/02/2008 16:15

I had a reading that was 2 the first month i had my bloods done, a fww months later it was 32 and a few months after that we conceived. Sadly i have just lost that baby at 13 weeks so am going to be back to TTC. The gynae i spoke to at the hospital said that it was unlikely that the low progesterone caused the MC and in any case they dont offer progesterone supplimentation. Anyway, i just though i'd let you know that a one off really low reading doesnt mean its always that low.

jd32 · 15/02/2008 16:24

Thank you! I just feel like I want to be doing something about it which is why I thought Clomid would be worth a go, going to see specialist tonight so am hoping he will let me try it

Pinkali37 · 26/02/2008 10:46

Hi All,

I'm on clomid, have just doen 3 months at 50mg and last months blood test was at 1.0 (not good when it was more without Clomid), have just had day 21 test today and am booked in for a further 2 (day 23 and 27). If it doesn't work this month will be going up to 100mg so to answer your question JD32 Nope can't see that it would hurt. Obviously there are the usual risks attached such as hyperstimulation of the ovaries but that is very rare!! The hot flushes are great fun . I also had Mc in October last year...

xx
