HELP! Subfertility Tests: LH Peaked, 21day Progesterone 53 n/mol, No ovulation seen on scan

Long time lurker, first time posting here, hi! waves

I'm currently having subfertility tests, trying for our second, I've had 3 miscarriages in the past year.

Cycle length: 29days
OPK Peak: day 17
Internal scan: day 24 (7 days since peak so assume 6dpo??)
I had an internal scan and the sonographer said one ovary looked "good with lots of eggs" the other was "borderline polycistic". She also said she couldn't see that I had ovulated this month. I was obviously a bit surprised by this and a little sad.

Progesterone: I had my progesterone tested 7 days before my period is due (actually 6 days as 7 was a Sunday!) result: 53.2 n/mol which, on the print out I was given is > 25 n/mol "worry zone"

From everything I have read, if my progesterone had been "low" then it would have confirmed that I didn't ovulate. But my progesterone was good to high. Could the sonographer have missed this? Don't know what is right and my followup consultation is more than a month away!

Any advice/ experience welcome!

Please help me make sense of this!

I think it can be quite difficult to see everything on a sonograph, it will depend on positioning of your ovaries and is liable to misinterpretation. Your blood tests indicate you did ovulate. Do you have any reason to think you are not ovulating? Do you track temperature at all?

Getting a positive OPK, having regular cycles, and having a good progesterone level at cd21 certainly indicates you are ovulating normally. My progesterone was 30 and I'm pretty sure I ovulate.

Sorry for your losses. Have they all been similar? How far along did you get in each?

Hi @79andnotout sorry it's taken me ages to figure out how to reply Hmm

They're looking to see if I do ovulate as an all over MOT as we've been TTC every month and since the last miscarriage (last summer) we haven't conceived. I do get "high" and "peak" clearblue test results every month (although these are potentially a bit later than would be expected from a regular 29 day cycle) so I had assumed that I definitely was ovulating. I haven't ever done temps. It was only when the sonographer said that she couldn't see that I had this month that suddenly it was all thrown into doubt in my mind!

I would have thought so too but now I'm confused!!!

First was at 9.5 weeks. It was a very long drawn out situation which ended with an ERPC 4 weeks after the original "diognosis". Second was 5 months later, 7 weeks and natural. Pregnant on the following cycle. Third was a suspected ectopic at 7.5 weeks. Again all happened naturally.

HI @cha77erbox, that all sounds quite traumatic. Doesn't really sound like ovulation is your problem though, as you do get pregnant (and last summer wasn't that long ago). Hope they find a reason for the miscarriages, or that your next pregnancy is okay. A friend of mine just had three miscarriages in a row and is now quite far into a healthy pregnancy.

Thank you @79andnotout my DS was conceived second month of trying and 1st pregnancy after was before my periods restarted after 10months breastfeeding. Everything since then has been a mystery! Hoping we might have some answers soon! Thanks for your opinions x
