PCOS ttc third baby

I am 30 years old and was diagnosed with PCOS at 23. I was told then that I would struggle to get pregnant. Luckily and thankfully I have been blessed with two daughters now aged 4 and 18 months. Both we conceived naturally, and quickly.
In february 2017 I suffered an early miscarriage between 5 and 6 weeks.
Since the miscarriage my periods have been very irregular, and though I am aware that this is a symptom of PCOS, I was getting a period every month so there definitely has been a change.
We have been having unprotected sex since May however we have only decided to actively try for a third baby since July.
I am worried that it isn't going to happen easily this time. Should I tell my doctor that we are trying, In case it doesn't happen easily. I'm scared that if I do conceive again ok will suffer another miscarriage.

I don't know if I'm looking for reassurance or advice or just to put my fears down in black and white. Thanks x
