Enlarged ovaries and too many follicles?

Hi all

I have just started ttc around 2 months ago after having my nexplanon implant removed.

I have been having pelvic pain so doc sent me for ultrasound. I was told that i had enlarged ovaries with too many follicles but my lining looked healthy (so doc didn't think it was pcos at the moment). The sonographer said I wasn't ovulating.

I was told by the sonographer that i could ask doc for clomid to kick start ovulation or wait it out as it could be as a result of the implant. I am inclined to wait it out rather than pump more chemicals into my body.

I know its unlikely but has anyone else experienced this?

Well I'm not a dr but it does sound very much like PCOS to me. The good news is that ovulatory problems are usually the easiest to fix. PCOS is very common so there are plenty of posters here who have experienced it.

Personally, I'd want to wait and see for a bit as it is very early days. What were your cycles like before the implant?

I agree pcos but you would need blood tests to confirm. Some people just have cysts and nothing else.
What i would do would depend on age and waiting lists etc

Thanks both. I am 25 now and been on some form of contraception since i was around 18. I did have irregular periods before that.

I wouldn't be surprised if it was pcos but the sonographer recommended waiting to see what happened. He said there were no cysts but lots of follicles (i thought they were one and the same!).

I might go and see GP and ask him what he thinks but they usually like to tell you to wait and see....

Maybe he means that he has not identified a huge cyst that might need removing. The cysts in pcos are actually follicles, too many can start growing.
Other symptoms excess hair weight gain.
At your age you have time to have a go and see what happens, some people with pcos get pregnant naturally.
But weight loss (if excess) is often helpful.

Enlarged ovaries with cystic follicles are classic of polycystic ovaries.
Did they describe your ovaries as having a ring of pearls appearance to them?. Also polycystic ovaries are larger than a normal sized ovary which is about the size of a walnut.

Doubt very much the implant has caused this to arise seeing as you had irregular periods prior to using it. The implant basically masked the symptoms of the underlying problem.

Clomid should actually be prescribed only by a gynae consultant as it is quite powerful stuff and should be given to PCOS patients with a degree of caution. It can increase hormone levels markedly which is not desired at all.

I would monitor your pelvic pain and that is either cyclical in nature or increases in severity I would be asking now for a referral to a gynae. Such problems too are outside a GPs general remit.

PCOS is a very individualistic disorder and affects each woman with it very differently. The commonality amongst all though are the cystic follicles on the ovaries. These often disappear, only to be replaced by further cystic follicles. Some of these follicles are dormant and others contain hormones, one may have an egg.

Have you as yet had any blood tests done. You need a day 2 test to compare your LH level against that of your FSH level. in PCOS the level of LH is often elevated compared to FSH.

Thanks everyone - very helpful. I had a look at my ovaries on the screen and they didn't have the traditional ring of pearls that I have read is associated with pcos. I'm also fairly skinny but I know it's possible to have lean pcos.

Unfortunately not had a period yet since coming off implant on 18th dec so wouldn't be able to have day 2 bloods (I don't think??). I think I will go back and visit my gp to discuss the results of the ultrasound and maybe ask for a general blood test to see if that tells us anything.

Not sure if they would refer me to anyone due to only ttc for a short time and due to my age. I guess I can only ask!

Yes you can be skinny with it (my bmi has been 20-22)
I agree too soon but you should be able be be diagnosed as pcos (if you are) even if not ttc as it can affect other things as well as fertility.
