LH surge but no ovulation

Can anyone explain this? I've done my best with Dr Google, but haven't really found any information bar the fact that it may occur if you have PCOS. Why wouldn't an egg be released if your body gears up for it?

I had an LH surge once (enough to trigger a fixed smiley on the CB OPK), but no ovulation. I had just come off the pill, so my guess is the LH just wasn't strong enough, as the second LH surge a couple of weeks later was much stronger. But I think that as the LH surge happens 1-2 days before ovulation, if anything happens in that time (such as major stress), the body can still stop the whole process and not ovulate.

No idea! Maybe the surge wasn't strong enough? Or perhaps the oestrogen surge wasn't strong enough so the follicles were not developed enough? Sorry I can't help more! Hopefully the day 21 test will let you know what happened.
