TTC for over a year - what should we ask our doctor this week?

Hi

I am 35 and DH is 34. We have been TTC for over a year.
We have a 2YO DD after TTC for over 18 months.
Last time round we were referred to Guys (not our most local hospital but due to a family member link) and were undergoing tests to try and diagnose the prob. My hormone/blood tests came back fine but DH has low swimmers (15% rather than over 50% if I remember correctly) but then we fell pregnant Just before my laparoscopy and DH's repeat sperm test was due (no idea how or what we did differently that month!).

Given my age I am keen to restart investigations / help as there may be something we can do to boost DH's swimmers or something else we have overlooked.

We have an appt with GP this Fri and I was going to ask him arrange a repeat sperm test and a testosterone test for DH, but not sure what else. And also a referral to a fertility unit - not sure whether local hospital or back to Guys (who i have more faith in). I expect we will have to pay for treatment given we have DD - is that likely to include investigations in which case should I get more done by my GP?
